'Reducing inequalities in health is critical to achieving our aim of making Scotland a better, healthier place for everyone, no matter where they live. The Keep Well and Well North programmes of anticipatory care programmes is one example of how we can address this. This programme has now delivered more than 70,000 health checks to 45-64 year olds in our most deprived communities across Scotland and this year exceeded national targets.

'The health checks focus primarily on cardio-vascular disease (CVD) and its main risk factors, especially blood pressure, cholesterol, smoking and diabetes, but have also taken a whole person approach to patients, recognising their wider needs. Those found to be at risk are referred onto further services/brief interventions including smoking cessation, alcohol interventions and diet/weight management or are prescribed appropriate medications.

'The Cabinet Secretary for Health and Wellbeing announced in March that the Scottish Government will continue to fund the Keep Well and Well North programmes of targeted health checks after the end of the current funding period in March 2012. The programme will be extended to provide targeted health checks for all of the most deprived communities in Scotland.'
